Free Believe Curriculum Lesson

Medical Release Form
Once you've registered, download the medical release form and have everyone in your group fill it out (including all adults, sponsors and yourself.) You need to bring two sets of these forms to the event.
The first set is a copy of these forms for you to keep with you at the event. The second set is the originals that we will collect from you at registration. It helps both us and you if you can have the forms alphabetized by the person's last name before you arrive at the event. Everyone in your group must have this form to enter the event.
Feedback and Evaluation
The believe team values your feedback about our events. Please take a minute to fill out our online, post-event evaluation form. Please share any of your thoughts about your group's weekend at believe.
