• STEP 1 - Complete and Submit online Application Form
• STEP 2 - Three online Reference Forms from adults such as a minister, elder, youth sponsor, teacher, etc. A family member may NOT be used as a reference. (Send this link to each of your references, for them to fill out.)*
• STEP 3 - E-mail your 500 word essay based on the following (exempt for applicants 22 and older)*
  • Short autobiography
  • My qualifications to participate
  • Why I want to go on a mission trip
• STEP 4 - Mail $50 Application Fee and Discipline, Liability and Medical Release Form to CIY. (This download requires Adobe Acrobat Reader. Click here to download the program.)

* If you've been on a CIY mission trip before, you do not need to turn in the references or the essay (only complete Steps 1 and 4).

When downloading Reference Forms, print 3 copies. Please ask each person to complete their form, seal it in an envelope, and return it to you.

Group Contract

Do you want to bring a group of people on a trip, but you’re not yet sure who can come? A youth minister (or any other group leader) can reserve spots on specific trips (based on availability) for 30 days by submitting a group contract and a deposit of $100 per person. Doing this gives you some additional time to allow your group to complete their applications while the spots are being held. After 30 DAYS, any spot for which CIY has not received a completed application from each student will be considered “non-reserved” and will then become open to anyone.

Minimum group size must consist of 10 students or adults. The male/female ratio should be of equal proportions, or of no more than two-thirds female.

All deposits for accepted applicants will be applied to the cost of their trip. All other deposits are non-refundable and non-transferable.

Note: All applicants under a group contract must still complete all of the individual application requirements listed at the top of this page.
